Care at Every Level
Every resident has different needs, from requiring assistance in managing medications to help with daily living activities such as bathing or food preparation. It can be challenging to understand what exactly you or your loved one needs and match it to what a senior living facility offers. Our knowledgeable, friendly staff will help each resident achieve a level of independence that fosters a sense of engagement with life but provides a safe and secure setting as well.

Care Levels
Level I
Room, oral medication management, vital sign and weight monitoring, three meals per day, housekeeping services, activities, cable, WiFi, utilities (except phone).
Level II
All Level I amenities plus laundry service and bathing assistance.
+ $300 - $600 per month depending on frequency.
Level III
All Level I and II amenities plus incontinence\bathroom management, dressing assistance, hygiene assistance, one-person assistance with transfer\standing, mobility assistance, dementia care. + Additional costs determined on individual needs.
Level IV
All amenities listed plus total care, end of life care, special equipment, two-person assist with transfers, life transfer. This level of care requires the resident be on hospice services.

Additional Services
Diabetic Management
Diabetic management or other injections, wound care, special equipment or treatments (ie oxygen, tube feeding) are subject to an additional $300-$600 per month depending on frequency and time provided.
Community Fee
A one time $750 non-refundable fee prior to moving in to cover normal wear and tear on room and facility.
Additional Information
Move in - Upon moving into Oak Creek it is required that individuals are independent with mobility, most dressing, and are able to assist with bathroom management. As needs change, Oak Creek will add\adjust care and services per the levels of care. Rent rate will be increased accordingly, a 30-day notice of rate change is not required for a level of care change.
Couple living in the same room - additional $1,000 on room are plus level of care fee.
Pets - Additional pet fees of $500 per pet (non-refundable) are due before moving in for approved pets. Please discuss pet options with staff before move in. Oak Creek must approve the pet and ensure the resident has the ability to property care for the pet.
Oak Creek can work with residents who are on or will be going on the Medicaid\HCBS program. Residents who move in as private pay and will be transitioning to Medicaid\HCBS MUST GIVE OAK CREEK AT LEAST SIX (6) MONTHS NOTICE BEFORE THEIR TRANSITION. Failure to properly notify Oak Creek of your changing financial status and leading to a Medicaid pending status without the proper notice can lead to the resident receiving a 30 day discharge notice. Should this transition be necessary we want to work with you - communication and proper notice is KEY! We’ve worked with many Medicaid\HCBS residents - let us put our knowledge to work for you and assist you in finding the resources needed for your care.
Oak Creek may request to review financial and bank statements on a periodic basis.
